For ripper nothing beats DVD Decrypter or DVD FAB platinum, ripping is just one of the 100s tasks these appications can do. For conversion best application out there is VitrualDUB MOD. Though its kinda difficult to comprehend the options Vdub offers but it definately gives one of the best reasults out there. Refer some guide available online. I have used star Divx converter as well and was pretty happy with the result.

Eversince I stumbled on this tutorial http://www.thinkdigit.com/forum/showthread.php?t=32980&highlight=copy+DVDs
by go4saket, I have been using Ripit4me for ripping DVDs.

I have also been using the licensed version of DVDFab Platinum.I think that the free trial version has some limitations. You may not perhaps be able to select individual chapters etc. So check on it before downloading.

For conversion, amongst the many options you may also try Mediacoder.Nothing like trying out the various options and finding the best one which suits you. Each one may have his own favorite.
