When you're running WinXP, you should use the DirectSound output plug-in instead of waveout.(make sure you've not unchecked it in the installer). Waveout is outdated and uses very old APIs.
Also waveout has less features then DirectSound and probably more bugs.
Also make sure you're using latest drivers for soundcard and chipset.
