Want to create Feedback form.........?

I want to create Feedback form for my site
i have domain name & using free space from awardspace.com
I need codes to create feedback form

like
if some one fill up the form & press send, than it mail the whole detail to my email id & that person gets auto response

I searched many thread in digit to

but all the thread were useless
i dont know why all the way unable to send mail

awardspace.com suckz they there is no SMTP, so you cannot send email...

Advice :-Spend some bucks get paid hosting........

No need to buy hosting
register @ http://www.frihost.com/ or http://ej.am/ both give SMTP 4 free! with PHP & MySQL support and much more(POST 2 HOST)
do atleast 10 quality post request hosting ( i am using frihost)

but these free option are not garunteed stufff...................u can face frequent downtime and even data loss .....

not agree wid you. frihost gives you 98% uptime.

now I am using a hosting provided by a namepros member(forum) for only 50NP$ P/M(NP is the forum curreency) very easy to earn 50NP$ every month(HIGHLY RECOMENDED)

Well, frihost had their server 2 crashed sometime back, which resulted in the data lost of all users hosted in server 2. I was one of the affected.

98% uptime is I think, very poor. Good hosting companies provide 99.99% or 99.9% uptime. Any thing below 99% uptime is I think not good.
