Default puzzling 691 error question

hi,
i have a very annoying problem that has been driving me crazy for the past five days. i have a sify dial-up connection(one of those 3months deal for Rs.399.) for the past three years which gave me no problems until now. every time i try to connect to the net, i get a error 691 which says that my username and/or password is incorrect. but this can't be true as i've checked both very carefully and have accessed my online account(from another computer) using them.
sify customercare hadn't been of much help except to give me the

usual solutions regarding error 691 which i'd already tried.
i have two modems that i 've tried separately - a DFM-560es and a

dfm-562e++. both seem to be working with the bsnl isp but give 691

error with the sify one.
i've reinstall the drivers which i've downloaded from dlink site.

then i even reinstalled the os(win xp with sp2). still nothing.
what's going on? what am i doing wrong? can anyone help?
