Default want to forward port

how would i know what is my rooter address nd its model no, i had a cable inter net connection, where they donot provide any modem, only a cable wire fit in my lan connection, so if i want to forward a port, then how would i do this............

and i guess as u have cable net conection u mi8 not be using router...ask ur cable wala...
