How old is the disk? More than five years?
The problem may be with the OS. Repair the operating system using installation disk and tell the results.

Boot with your windows setup. Now instead repair option go through setup option.
you'll now get the option to partition & format existing hard disk.

dude go with normally, and then check if it comes back again.
If not well and good.
But if it comes again, then boot again using normally. then put the setup disc when inside window and do the repair
